Along with the task instructions, you will receive stimulus material (case notes and/or other related documentation) which includes information to use in your response. Speaking sub-test The Speaking sub-test consists of two profession specific role-plays and is delivered individually. It takes around 20 minutes to complete. In each role-play, you take your professional role (for example, as a nurse or as a pharmacist) while the interlocutor plays a patient, a client, or a patient’s relative or carer. For veterinary science, the interlocutor is the owner or carer of the animal. The Speaking sub-test has the following structure: In each Speaking test, your identity and profession are checked by the interlocutor and there is a short warm-up conversation about your professional background. Then the role-plays are introduced one by one and you have 3 minutes to prepare for each. The role-plays take about five minutes each. You receive information for each role-play on a card that you keep while you do the role-play. The card explains the situation and what you are required to do. You may write notes on the card if you want. If you have any questions about the content of the role-play or how a role-play works, you can ask them during the preparation time. The role-plays are based on typical workplace situations and reflect the demands made on a health professional in those situations. The interlocutor follows a script so that the Speaking test structure is similar for each candidate. The interlocutor also has detailed information to use in each role-play. As you are 25 years old, you feel quite upset about the problem. You are embarrassed to be here because you feel like only old people consult podiatrists. You are going on a university beach trip in two weeks, and you want it cured quickly so it won’t be seen on the beach. • Find out what the problem is and tell the podiatrist that you thought this only happened to old people. • Ask what can be done and explain the urgency. • Tell the podiatrist your parents used to buy your shoes, but now you are a university student and you do not have the funds or time to have them fitted correctly. • Tell the podiatrist that you are worried about pain or being cut if the corn is removed with a scalpel. • Ask how you can avoid this happening again. • Ask the podiatrist how hard you should rub with the pumice stone. Sample role-play NO. 2 PODIATRY Sample role-play Podiatry Clinic PODIATRIST Your patient is a 25-year-old university student who feels quite upset about having developed a clavus (corn) on the right big toe (R1). There is a university beach trip planned in two weeks and he/she wants the corn cured quickly, so it won’t be noticed by friends on the beach. The patient seems very embarrassed. You consult the patient sensitively. Sample role-play • Highlight the fact that a clavus (corn) occurs when there is extra pressure on the toe. • Tell the patient you see a lot of younger people with clavi (corns). • Find out the patient’s approach to buying shoes. Explain the importance of having good shoes that are fitted correctly. • Advise that the clavus (corn) can be immediately excised by scalpel. Find out if the patient is agreeable to the procedure and reassure them that it is painless and carries minimal risk. • Reassure the patient and explain how to avoid a recurrence of the problem. • Provide the patient with a pumice stone and explain how to use it properly. 21www.occupationalenglishtest.org Diagnosis: Treatment: R 2nd toe – flexible hammer toe deformity with overlying heloma durum (corn) Conservative (initially) including: • debridement of corn (routine) • footwear changes − correct fit, spacious toe box (possibly open toe shoes when suitable or runners/sneakers) • digital padding Recommended several shoe specialists with range of mens larger shoe sizes 44–50 including podiatrist designed lines If not successful − surgical options (e.g., tenotomy) Writing Task: Using the information given in the case notes, write a letter to Dr Spring, the doctor who referred the patient, informing her of your diagnosis and treatment.
